Quick Document Certification Guide for Malaysia <> AU AHPRA (2025)

This guide summarizes the latest requirements and procedures for document certification as outlined by AHPRA as of 2025. Ensure you follow these guidelines carefully to meet the registration document requirements.

For Applicants

  1. Make a Copy:

    • Photocopy your original document.

  2. Find an Authorised Officer:

    • Bring both the original and the copy to an authorised officer.

    • Acceptable officers in Australia include:

      • Registered health practitioners (e.g., medical practitioners, nurses, midwives, pharmacists, etc.)

      • Public servants (employed for 5 years or more)

      • Teachers at schools or tertiary institutions

      • Legal professionals (e.g., lawyers, patent attorneys)

      • Court officials (e.g., justices of the peace, judges)

      • Commissioners for Affidavits or Declarations

      • Elected government representatives

      • Bank or finance officers (employed for 5 years or more)

      • Veterinary surgeons, accountants, ministers of religion/marriage celebrants, and other listed professionals

    • Outside Australia, authorised officers include notaries public, legal professionals, Australian registered health practitioners, consular or diplomatic officers, etc.

  3. Certification Requirements:

    • The authorised officer must certify your copy in person (no remote certification via audiovisual links).

    • They will:

      • Verify that the copy is identical to the original.

      • Certify the document using the required wording:

        • Single-page document: “I have sighted the original document and certify this to be a true copy of the original.”

        • Multi-page document: Initial every page, number them (e.g., page 2 of 5), and include the above statement on the first page. If both sides require certification, the statement must appear on both.

    • For any documents containing a photograph (e.g. a passport, driver’s licence, etc.), they will certify the documents with the statement “I certify that this is a true copy of the original and the photograph is a true likeness of the person presenting the document as sighted by me.”

    • Important: Incorrectly certified documents will not be accepted and may cause delays.

For Authorised Officers

  1. Document Inspection:

    • Confirm that the original document is authentic.

    • Check that the provided copy matches the original exactly.

  2. Certification Process:

    • For a Single-Page Document:

      • Sign and date the copy.

      • Write or stamp the following statement:
        “I have sighted the original document and certify this to be a true copy of the original.” OR “I certify that this is a true copy of the original and the photograph is a true likeness of the person presenting the document as sighted by me.”

    • For Multi-Page Documents:

      • Initial each page and number them (e.g., “page 2 of 5”).

      • On the first page, include the certification statement.

      • If certification is needed on both sides, ensure the statement appears on each side.

  3. Include Your Details:

    • Your signature and full name.

    • Your occupation or profession (include your occupation/profession number, if applicable).

    • Your stamp or seal (if applicable).

    • Your phone number.

    • The date of certification.
